Information for Professionals

Referrals can be made to Helen's Trust by any health care professional including:

• the GP
• district nurse
• practice nurse
• community matron
• health visitor
• social worker
• social services
• physiotherapist
• occupation therapist
• Macmillan nurse
• CPN
• Hospital or hospice doctors, nurses or other therapist etc.

Referrals cannot be made directly by a patient, but we do encourage patients and relatives if they identify a need to contact their primary health care team.

5 Questions to Identify who Qualifies for Help

1. Does this person have an incurable illness?
2. Would the medical professionals responsible for this person not be surprised if that person were to die within the next 12 months?
3. Is this person registered at a designated practice?
4. Will this intervention help this person stay in their home?
5. Have statutory services already been approached?

Designated practices, please see  "How can we help you"

A patient's home is their main place of residence and does include residential homes (but not nursing homes).

Statutory services include social services, community nursing teams, community physiotherapists etc., and these bodies must be approached first. It is also expected that Macmillan and Marie Curie will be approached first.
