The referral process

How can I access the support that Helen’s Trust offers?
To receive the support on offer from Helen’s Trust you need a referral from your health or social care professionals.

How does the referral process work?
Your GP, hospital consultant, nurse or another health/social care professional working in the community should contact us when they feel that the support you need could be provided by Helen’s Trust.


He or she will check if you meet our referral criteria, then contact our referrals co-ordinator who will co-ordinate the care or equipment that can be funded or provided by us.


The support we provide is tailored to meet the needs of an individual and will depend on the referral meeting our criteria.


What is the criteria?

We are hoping to expand the area that we cover to include the High Peak area of North Derbyshire following review in 2013. You must be referred by a health or social care professional.

  • You must have been diagnosed with an incurable illness.

  • The help requested must be to support you to stay at home.

  • Statutory services must have been approached to support you with care that they are responsible for providing.

  • Helen's Trust will need to discuss life expectancy with the professional making the referral.


How long will it take for support to be provided?

We aim to support people as quickly as possible. Our referrals co-ordinator will action the referral as soon as she receives it – she will also let the referrer know if we are unable to help you.


How can I contact  Helen’s Trust?
If we are able to support you, we will be in touch once help has started. We will send you a card with your referral co-ordinator's contact details. In the meantime if you have any questions please contact Sarah, via email or call 07780 331715


 We may also contact you to see how the support we have provided is helping you. This contact is important to us as a charity; it helps us to improve our service to you, as well as demonstrate why the work of the charity is so essential and it can help us to generate future donations and funding.
