Referral criteria
Any individual being referred to Helen's Trust should meet the following referral criteria;
- must be referred by a health or social care professional
- must have an incurable illness
- must have a likely prognosis of less than 6 months (assessed by a qualified doctor)
- referrals must be to help enable an individual to stay in their own home
- statuary services must have been approached first and be fulfilling their obligations
- You can refer an individual who is living in North Derbyshire or Sheffield, providing that they are registered with any of the following GP practices
Please note – this list represents phase 1 of our expansion plans, we hope to be able to extend our support to the High Peak area of North Derbyshire following review in 2013.
